I'm hoping someone can help me identify this snake species, found in Northern California (Napa County region).

Looks like a Western Diamondback rattlesnake (Crotalus atrox). More likely it could also be a Northern Pacific rattlesnake (Crotalus oreganus oreganus), which tends to live further north. The two look a lot alike but the extra white on the face and the northerly locale makes me think Northern Pacific (Diamondback has 2 vertical stripes on its face). Keep a respectful distance from it. They are venomous. Those species are feistier than some other species and will vigorously defend themselves if disturbed. As long as you give it space and leave it alone it will not hurt you.

ETA: If this snake lives in your yard I would suggest calling a professional to have it safely relocated. Don't try to do it yourself.
